Admissions Application Checklist

APPLICATION PROCEDURES CHECKLIST: Keep this checklist and use it for your personal reference. It is the responsibility of the parent to be sure all application materials are completed and received by the Admissions Office. The admissions process for your child cannot proceed until these materials are received. We need the following from you prior to your interview:

_____PASTOR’S REFERENCE

_____OFFICIAL BIRTH CERTIFICATE (we must see a certified copy)

_____IMMUNIZATION RECORDS (current physical)

_____STUDENT CLASSROOM VISITATION AND PLACEMENT ASSESSMENT SCREENING
